Tanning after Surgery

In my Louisville Plastic Surgery practice a common question is “When can I tan again?”  It seems like this question is most common after a Louisville Breast Augmentation or a Louisville Tummy Tuck.

When a surgeon wounds the skin by making an incision, the tanning cells, your melanocytes, are prone to produce pigment along the edges. This is particularly true for darker skin types. That is why some fresh scars are darker than the skin around them. If you get into a tanning bed and expose the wound to more tanning stimulus, it very well may result in your scar becoming hyper-pigmented (very tan).

My advice is to never get into a tanning bed, but if you are going to at least wait 6 weeks after surgery or keep your incision covered.

Drains and Plastic Surgery

If there is one constant in my Louisville Ky cosmetic surgery practice, it is the dislike of Drains.

Drains are round or flat tubes that are placed in surgery. They exit through the skin and empty into some form of reservoir. Patients don’t like them at all but they are necessary “evils” following procedures like Louisville Tummy Tucks and Louisville Facelifts.

During surgery if we elevate the skin, as in a facelift, breast lift, or tummy tuck from the underlying muscles, our body will leak serum into that new space. Serum is basically the watery part of our blood. If there is no method to evacuate that fluid, it will form a collection called a seroma. Seroma can cause a variety of problems and so we do our best to avoid them. Thus,  the drain. The drains are placed and serve as an exit route for the fluid. They stay in after surgery anywhere from 12 hours to a week or two depending  on the procedure. They are typically removed not based on time but on output.

So, while they are not the most popular part of cosmetic surgery, they do serve a valuable purpose in helping you achieve a great surgical result.

Complications and Fillers

Injectible fillers in Louisville, such as Louisville Juvederm and Louisville Restylane, are some of the most commonly requested services requested in my Louisville Cosmetic Surgery practice. In the vast majority of cases, the injections are well tolerated with good results and happy patients.

Fillers have been extensively tested and are very, very safe. Most problems are actually not a result of the filler itself, rather a problem with injection technique.

Early problems are the most common ones we encounter and include redness and swelling. Bruising can occur if a small blood vessel is injured by the injection. With the exception of fat injections, an allergic reaction can occur with any filler but this is extremely rare with todays fillers.

Late problems are primarily related to granuloma formation. A granuloma is basically a scar reaction to the injected material. If this occurs it is typically 6 to 24 months after injection.

Combining Cosmetic Procedures

Here in my Louisville Plastic Surgery office we are seeing our usual “Spring Rush”.  More than ever, it seems like patients are wanting to do more than one operation simultaneously. Common examples are a Louisville facelift with a blepharoplasty or a Louisville Breast Reduction with a Lousiville Tummy Tuck.

My opinion on combining procedures is based on many factors. First, I look at your overall health status. Secondly, I look at the scope of the procedures we are planning and their physiological impact, and finally we need to consider how much time it will take to get you in and out of the O.R.

My advice is to come in for a consultation, let me see your “wish list”, then we’ll take a look at your starting point. I can then map out a plan and we can then decide the feasability. Our consultations are complimentary.

2009 Cosmetic Surgery Stats

I received the stats from the American Society of Aesthetic Plastic Surgeons yesterday for 2009.

The most commonly performed surgeries for women were 1) Breast Augmenation 2) Liposuction 3) Blepharoplasty 4) Tummy Tucks and 5) Breast Reduction.

In my Louisville Plastic Surgery practice my most commonly performed surgeries were 1) Breast Augmentation  2) Liposuction  3) Tummy Tuck  4)  Breast Reduction and 5) Face Lifts.

Maintenance of Board Certification

The American Board of Plastic Surgery requires all member surgeons certified after 1995 to participate in a Maintenance of Certification process. Older surgeons who were certified before 1995 are grandfathered in.

The maintenance program measures a surgeon’s dedication to continuing education. As a Board Certified surgeon who got his certificate in 1999, I participate in the MOC process. What this involves is scrutiny on the Boards part of my cases, patient satisfaction, professional standing with the hospitals and state licensure board. In addition, I have to take and pass a written exam every 10 years to keep my Boards. I took the test early in 2007 and am certified through 2019.

When you see a Louisville Plastic Surgeon ask if they participate in the MOC process.

American Board of Plastic Surgery

While seeing patients today in my Louisville, KY Plastic Surgery office, I received the newletter from the American Board of Plastic Surgery.

On the cover was a photo of the Chairman of the Board, Dr. Michael Sadove. Also  newly elected to the Board was Dr. Robert Havlik. Both of these accomplished surgeons and national leaders trained me during my Plastic Surgery residency!

In addition, our Department Chairman, Dr. Jack Coleman is Past Chair of the American Board of Plastic Surgery.

Congratulations  to my former teachers, Drs. Sadove and Havlik!

Swelling after Surgery

After cosmetic surgery procedures such as a Louisville Breast Augmentation or Louisville Liposuction, there is soft tissue swelling. This is a normal sequela.

Why?

As far as our soft tissue know, surgical trauma to our tissues is no different than any other kind of trauma. Our tissues are wounded and they healing by what is called an inflammatory repsonse. The normal inflammatory response results in soft tissue swelling.

The swelling, depending of the parts of the body treated and the surgery, can last anywhere from a few days to weeks to even months. This is not only normal following surgery, it is to be expected.

Cosmetic or Insurance?

When I consult with a new patient as a Louisville Plastic Surgeon, their problem is deemed either “cosmetic” or “insurance”.

These designations refer to with whom financial responsibility falls. If a case is considered cosmetic, the financial responsiblity falls with the patient. Insurance will not be billed. There is no fee for these visits. Cosmetic cases are typically Louisville Breast Augmentations, Louisville Tummy Tucks, Louisville Liposuction, Louisville Rhinoplasty, and Louisville Facelifts

If a case is considered reconstructive and the patient expects their insurance to cover the costs of the surgery, their insurance will be billed. In these cases there IS a charge for the consultation.
