Nipple Sensation after Surgery

As a Louisville Plastic Surgeon, a common question from women considering a Louisville Breast Augmentation or from men considering a Louisville Breast Reduction is the issue of nipple sensation after surgery.

Contrary to what may seem intuitive, an incision at the areolar edge does not lead to long  term nipple sensory loss. This is true for a female breast augmentation or breast lift and a male or female breast reduction.

The reason that you loose sensation to the nipple area on a permanent basis stems from an injury to the 4th Intercostal Nerve. This nerve lives deep within the breast on the rib cage surface. It is not affected by a skin incision but rather the deeper maneuvers Louisville Plastic Surgeons make to perform your breast surgery.

Your breasts are NOT droopy…

I’ve heard this complaint thousands of times in my Louisville Cosmetic Surgery office and a lot of the time the breasts are not droopy…technically.

The fact that the upper part of the breast has lost its fullness with age and babies does NOT constitute droop in Plastic Surgery terms.

Droop, or Ptosis ( toe-sis)  as we call it, is all based on where the nipple rests in relation to the crease under the breast (Infra-Mammary Fold or IMF). By definition, if your nipple sits above your IMF you do not have ptosis and a breast lift or Mastopexy may not be needed. A Louisville Breast Augmentation will often correct this situation.

If your nipple sits at or below your IMF you do have ptosis and a Louisville Breast lift will be necessary in most cases.

Now, a lot of times her nipple is well situated above the IMF but the bottom part of the breast sits below the IMF. We call this pseudo-ptosis and depending on the severity and the size of your breast, this situation may also call for a mastopexy (lift).

My Breasts are different sizes…

In my Louisville Kentucky Cosmetic surgery practice I encounter this complaint frequently. And that is no great surprise becasue every woman’s breasts are uneven.

When a woman is considering a Louisville Breast Augmentation, I will frequently use different volumes to account for the asymmetry or even different style implants. I tackle the size problem  by using implant sizers. These are trial implants that we place and then fill to different volumes to see what balances things out. They only run $30 or $40 which is a lot better than trialing a $1000 silicone implant only to find its the wrong size!!

When I do Louisville Breast Lifts or Louisville Breast Reductions, I can simply remove more tissue from the larger breast to balance things out.

What is more difficult to correct is if the breasts are vertically asymmetric. It is hard to adjust the breast crease a lot so these asymmetries tend to persist. Likewise, if your rib cage is different from side to side, there is very little to do.

Tanning after Surgery

In my Louisville Plastic Surgery practice a common question is “When can I tan again?”  It seems like this question is most common after a Louisville Breast Augmentation or a Louisville Tummy Tuck.

When a surgeon wounds the skin by making an incision, the tanning cells, your melanocytes, are prone to produce pigment along the edges. This is particularly true for darker skin types. That is why some fresh scars are darker than the skin around them. If you get into a tanning bed and expose the wound to more tanning stimulus, it very well may result in your scar becoming hyper-pigmented (very tan).

My advice is to never get into a tanning bed, but if you are going to at least wait 6 weeks after surgery or keep your incision covered.

Breast Implants and the Dentist

A fairly common question posed after a Louisville Breast Augmentation is should you take antibiotics before dental procedures. The ratinonale is that when the teeth are manipulated, bacteria get in the blood stream and if they find the implant they could result in an infection and loss of the implant.

In reality, the chances of this happening are very, very low. As a rule I do not put my patient’s on antibiotics before a trip to the dentist but there are exceptions. If the implants were placed within the previous 6 months I think it’s not a bad idea to take a dose before the dental appt. I also recommend them for my patients who have implants for breast cancer reconstruction. These patients may have weakened immune systems and are more prone to infections. In 12 years I have only had 1 patient get an infection after dental work and she was a cancer patient.

The bottom line is that there are no definitive studies that answer this question. If you have breast implants and are concerned it is best to discuss it with your surgeon.

Drains and Plastic Surgery

If there is one constant in my Louisville Ky cosmetic surgery practice, it is the dislike of Drains.

Drains are round or flat tubes that are placed in surgery. They exit through the skin and empty into some form of reservoir. Patients don’t like them at all but they are necessary “evils” following procedures like Louisville Tummy Tucks and Louisville Facelifts.

During surgery if we elevate the skin, as in a facelift, breast lift, or tummy tuck from the underlying muscles, our body will leak serum into that new space. Serum is basically the watery part of our blood. If there is no method to evacuate that fluid, it will form a collection called a seroma. Seroma can cause a variety of problems and so we do our best to avoid them. Thus,  the drain. The drains are placed and serve as an exit route for the fluid. They stay in after surgery anywhere from 12 hours to a week or two depending  on the procedure. They are typically removed not based on time but on output.

So, while they are not the most popular part of cosmetic surgery, they do serve a valuable purpose in helping you achieve a great surgical result.

Combining Cosmetic Procedures

Here in my Louisville Plastic Surgery office we are seeing our usual “Spring Rush”.  More than ever, it seems like patients are wanting to do more than one operation simultaneously. Common examples are a Louisville facelift with a blepharoplasty or a Louisville Breast Reduction with a Lousiville Tummy Tuck.

My opinion on combining procedures is based on many factors. First, I look at your overall health status. Secondly, I look at the scope of the procedures we are planning and their physiological impact, and finally we need to consider how much time it will take to get you in and out of the O.R.

My advice is to come in for a consultation, let me see your “wish list”, then we’ll take a look at your starting point. I can then map out a plan and we can then decide the feasability. Our consultations are complimentary.

Keller Funnels

I blogged about his topic a few days ago but I wanted to stress how much easier the keller funnel makes Louisville  breast augmentations.

The funnel looks like the bags used to apply icing to cakes. The way it works is I am able to drop the silicone implant into the funnel, place the smaller opening of the funnel thru the incision into the implant pocket. Then I just squeeze and the implant goes in. This keeps it from touching the skin and getting contaminated. Skin bacteria are thought to be one of the causes of scar capsule formation. Thus, the hope is that using the funnels will help reduce capsular contracture rates.

Better but still the same Breast…

When I meet a new patient considering a Louisville Breast Augmentation, a Louisville Breast Lift or a Louisville Breast Reduction they often share a common misconception. That is, after one of the aforementioned procedures, their breast tissue will change. This isn’t the case.

After an augmentation the breast will be larger. The upper volume will be restored. Droop can be corrected. BUT…the breast tissue that sits on top of the implant is still the same breast tissue you started with. It will not become firmer. The skin will not change. Stretch marks will not go away etc…

The same is true for a reduction or lift. These are great operations because they work! But, again, the nature and consistency of the breast tissue itself is unchanged.
